1. Symptoms (1) The incubation period is 3 to 32 days, with an average of about 7 days. The clinical symptoms can be divided into three stages. (2) The first stage is mainly manifested as chronic migratory erythema of the skin, which is seen in most cases. At the beginning of the disease, symptoms such as fatigue, chills, fever, headache, nausea, vomiting, joint and muscle pain are often accompanied, and meningeal irritation may also occur. Local and systemic lymph nodes can be enlarged. Occasionally, splenomegaly, hepatitis, pharyngitis, conjunctivitis, iritis, or testicular swelling may occur. (3) In the second stage, a few weeks or months after the onset, 15% and 8% of patients have obvious signs of neurological symptoms and cardiac involvement, respectively. (4) Stage 3 Within a few weeks to 2 years after infection, about 80% of patients have joint symptoms of varying degrees, such as joint pain, arthritis, or chronic invasive synovitis. Large joints such as knees, elbows, and hips occur frequently, and the surrounding tissues of small joints may also be affected. The main symptoms are joint pain and swelling, and a small amount of fluid in the knee joint. Often recurrent attacks. 2. Signs (1) Skin lesions are often the first symptom, characterized by chronic migratory erythema, initially as red macules or papules, and gradually expanding into ring-shaped lesions. (2) Nervous system disease Approximately 15% of patients, appearing at the same time as the rash or 1 to 6 weeks after it subsides. Manifestations include meningitis, cerebroneuritis, chorea, cerebellar ataxia, meningeal irritation, coma, facial paralysis, or trigeminal neuralgia. (3) Heart disease is seen in about 8% of patients. Atrioventricular block, myocarditis, pericarditis or pancarditis often occurs 3 weeks after the appearance of the skin lesion. (4) Arthropathy is seen in about 60% of patients. Large joints are involved, especially knee joints, with repeated episodes of swelling and pain. 10% of patients can turn into chronic arthritis. (5) Other manifestations: fever, fatigue, myalgia, nausea, vomiting, conjunctivitis, iritis, lymph nodes and hepatosplenomegaly.
The peripheral blood picture is basically normal, the erythrocyte sedimentation rate is slightly increased, the cryoprecipitated immunoglobulin in the serum may be positive, and the transaminase may be increased.
